It is our policy that any payment (including co-pays and old balances) is required at the time services are rendered. The parent, grandparent or adult accompanying the patient is responsible for this payment. Our office will bill your insurance plan for all office and hospital charges. You are responsible for any charges that are not 100% covered by your insurance plan which we will balance bill you in a monthly statement. For your convenience, we accept cash, checks, Visa, MasterCard, Discover and American Express.

You are required to bring your child’s insurance card to each visit.
